The Ford Transit's OBDII port may stop working if wiring to the port has become disconnected or compromised or the van has blown a fuse. Test by plugging the scan tool in a computer to see if it works using a different electrical source. The interior light fuse #72 powers the OBD. Replace to see if the issue is corrected. Alternatively, if the electrical plug and wiring are operative, the port assembly may need replaced. The Ford Transit's OBDII can usually be located under the steering wheel on the left-hand side lower dash panel. The fuse box is accessible by removing the dash panel below the steering wheel. It is housed to the right of the driver's knee and should contain detailed diagrams for plug placement.
