A spark plug may pop off, causing engine issues. You can have oil leaks and those need to be repaired as soon as possible. There could be excessive oil use, requiring the oil to be replaced sooner than expected. A really serious problem is the engine overheating. This may lead to fires and the car exploding.

Typically the belt needs changing every 10 years or between 60,000 and 80,000 miles or if notice any cracks in the belt or tooth missing, best bet is check with your manual, as for the other 2 guys answers saying 150,000 miles is rubbish, thats just hoping for you to get stuck on the roadside somewhere with a broken engine. Timing belts or chains rely on regular checks or replacement along with oil changes. Each chain or belt has a built in safety factor of probably 10%. However regardless of this and to avoid belt driven engine failures replace a lot sooner than the manufacturers spec as for chains you can hear the noise before it snaps belts just collapse causing endless damage.
