There are a number of ways and reasons that a vehicle's USB port may fail. In the case of the Chrysler Pacifica, it's hopefully pretty straightforward -- however, You've first got to clarify what You mean by "fail". The first things You should check to ensure are that Your device or Your cable are not to blame. The cable will be the cheapest fix You can have, and I can't tell You how many times I've had a MINI heart attack worrying about my USB port when it was just a cable. Alternatively, You might have either a data stream issue, or an overall power issue. More than likely, either of these are caused by wiring or other hardware failure. In this case, it's cheapest just to swap out the entire port for a new one.

USB port issues on the Chrysler Pacifica are usually caused by a blown fuse or loose wiring.

Following solution worked for 2020 Chrysler Voyager minivan. The USB port next to the AUX port on the dashboard suddenly stopped working, no charging of the phone or connecting to Android Auto, AUX was working fine. Assuming the fault was caused by "unplugging of USB connector while Android Auto is still active". Steps taken to resolve the issue - 1) Switch off the van. 2) Open the hood. 3) Pulled out fuses 28 & 76 from the fuse box on the driver side near the hood & body joint. 4) With hood open, fuse box cover left outside the engine, started the van and switched off after a minute. No one suggested this step, but I did it anyway to make sure UConnect remains off while the van is on. 5) Tested both the fuses with a multimeter to make sure they were not blown. 6) With van switched off, re-inserted both the fuses (# 28 & 76). 7) Closed the fuse box with it's cover. 8) Closed the hood. 9) Removed the USB cable from the port on the dashboard. 10) Started the van. 11) Inserted the small end of USB cable (connector C) into the phone, while phone was NOT in sleep mode. 12) Inserted the bigger end / connector of the USB cable into USB port on the dashboard. Android Auto was launched automatically, immediately after inserting the cable into USB port on dashboard.
